Pharmacy

Doctors/Physicians/Nurse Practitioners or Healthcare Providers:

You may place an order via E-scribe, phone, fax or email. When phoning in, your call will be transferred to a pharmacist to who will take your prescription order. If you are E-scribing, faxing or emailing your order, please provide all the information possible for the medicine you would like us to fill by including the following details: prescribing doctor, drug, strength, quantity, dosage, and flavor when applicable – along with patient demographics including: patient name, D.O.B., insurance, and any allergies. Please notify us of any special requests. All prescriptions for controlled substances will be verified by a pharmacist and patients must have a valid, state issued photo identification to get these filled.

Patients Calling for Refills/Transferring Prescriptions:

You may place an order for a refill via phone, fax or email. When phoning in, if it is something you have never ordered from us before, your call will be transferred to a pharmacist to place your order. If the prescription is something you have gotten before and you are calling for a refill, you may place your order through any of our technicians or the pharmacist. (In this case, having your prescription number handy will expedite the process.) If you are faxing or emailing your order, please provide all the information possible for the medicine you would like us to fill including: the prescribing doctor, drug, strength, quantity, dosage, and flavor when applicable – along with your name, D.O.B., address and insurance information (if there are any changes please provide current and up-to-date information), and any allergies you may have. Please notify us of any special requests.

Note: All prescriptions for controlled substances will be taken by a pharmacist (patient must have triplicate-cannot call in) and patients must have a valid, state issued photo identification to get these filled. We will then contact the patient and coordinate delivery with them as per their convenience.
